We are a multi-faceted and fully accredited by the Community Health Accreditation Program (C.H.A.P.) agency providing individuals with disabilities the opportunity to safely live and remain in the community.

Whether supports are needed in the home, or in a group home setting, we can provide nursing assistance, home health aides, direct support professionals, or group home living, as well as transportation, respite, companionship, and many more services to support living in the community instead of a nursing home or facility.

We are an approved provider with NJ Medicaid for Private Duty Nursing (PDN); Personal Care Assistance (PCA); Home Health Aide (HHA); and Traumatic Brain Injury (TBI) services; with the major HMO’s, private insurances and direct pay. We are also an approved provider with the State of New Jersey Department of Human Services’ Division of Developmental Disabilities (DDD); The NJ Department of Aging Services’ Jersey Assistance for Community Caregiving (JACC) Program for seniors; and the Veterans’ Assist Program for veterans and their spouses.

Our new group home, “The Sunset House” is a beautiful 4-bedroom ranch located in Hamilton, NJ. From all of the furnishings to the appliances, everything is brand new in the home. We have an experienced staff to ensure our residents are comfortable and safe in their New Life home!
